Insurance and Billing
At Frontier Eye Care, we strive to make your life easier, including filing insurance claims on your behalf.
Please understand, however, that we may not have a contract with your insurance company. Also, failure to present your insurance information at the time of your visit will limit our ability to file a claim on your behalf. In such cases, we simply require payment at the time services are rendered, and will provide you with an itemized receipt that you can personally file with your insurance company.
And, of course, even if we file a claim on your behalf, you are still responsible for copayments, deductibles, and/or denied claims.
We currently file claims with the following Vision Insurance Plans:
- VSP (Vision Service Plan)
- Title-19 (Wyoming Medicaid) for patients under 21
- KidCare CHIP for patients under 21
We can also submit claims for eye disease or injury with most major medical plans including Blue Cross/ Blue Shield of Wyoming, Medicare and Medicaid.
If you have questions about vision or medical insurance or would like us to check your eligibility, please call our office at 307-472-2020.
New Patient Documents
If you are a new patient (or if we have not seen you in a while) we will have you fill out the Welcome/Patient Medical History form.
You can come to our office a few minutes early or, if you prefer, you can download the form by clicking on it below. Simply print out the form and fill it out at your convenience. You can then bring it with you to your appointment.
If you have Vision or Medical Insurance, you can do the same for the Insurance Information form. And if you feel the urge to review our Notice of Privacy Practices, you can click on that as well!
Flex Spending Acocunt
What is a Flex Spend Account?
A Flexible Spending Account (FSA), also called a flex plan or reimbursement account, is an employer-sponsored benefit that allows you to pay for eligible medical expenses on a pre-tax basis.
If you expect to incur medical expenses that won’t be reimbursed by your regular health insurance plan (including eye care, glasses, and contact lenses), you should be taking advantage of your employer’s FSA if one is offered, since it can save you money by reducing your income taxes. Talk to the Human Resources Manager at your office to see if your company offers an FSA.
Any expense that is considered a deductible medical expense by the Internal Revenue Service and is not reimbursed through your insurance can be reimbursed through the Flexible Spending Account. This includes fees paid for eye exams, contact lenses, and eyeglasses.
What is a Flex Spend Account?
You can use your flex spending dollars (tax free) at Frontier Eye Care. Depending on how your FSA is set up with your employer, you can either pay at the time of your visit (and get reimbursed by your account later), or use a flex spending card if you have one. We accept FSA cards at Frontier Eye Care, and will be happy to give you a detailed, itemized receipt to show diagnosis and treatment codes for your tax records.
